Types of incidents :
2.1
The equipment breakdown: production equipment is damaged or get malfunction so production is disrupted.
- Small breakdown: damaged equipment or production cuts, the actual loss under VND 1,000,000
- Average breakdown: equipment damage or production cut, the actual loss from the VND 1,000,000 to VND 20,000,000 .
- Serious breakdown: equipment damage or production cut, the actual loss more than 20,000,000 VNĐ
2.2
Work accident: during working process, employees get injured or acute poisoning
- Minor accident: need simple treatment or first aids, no need to treat by doctor in hospital
- Major accidents : victim's brain , eyes , limbs , body and other parts seriously injured resulting in working ability loss , need to take to the hospital for treatment or require leave for treatment
- Fatal accident: victim dies of the accident or during emergency aids
- Serious fatal accident: 1-2 victims die
- Especially serious fatal accident: more than 3 victims die
3. Produres of accident handling and investigation
3. 1
Once a minor accident or small breakdown of equipment happens, details of time, location, victim's name, and happenings shall be reported to Department Supervisor by victim or on-site witness. Department Supervisor shall report to General Affairs Dept., and company's responsible person.
3.2
Once a major accident or average breakdown of equipment happens, victims or on-site witness and related people shall immediately report to the superior or company's responsible person. Company's responsible person shall take the victim to hospital for cure.
3.3
Once serious breakdown or fatal accident or serious fatal accident or especially serious fatal accident happens, on-site witness shall immediately report to company's responsible person. Company's responsible person shall notify police, superior, Trade union,…
3.4
After the accident happens, the victim must be provided with first aid, measures to prevent accident recurrence must be taken, scene must be protected and follow the principle of "Four unforgivings" to seriously conduct investigation
3.5
Major accident or average breakdown of equipment is investigated and handled by Supervisor of General Affairs Dept in association with department of occurence and other related people.

Fatal accidents is investigated by inspector from Hanoi Department of Labour - Invalids and Social Affairs, police of Chuong My District, Hanoi Management Board of Export Processing and Industrial Zones, Trade Union of Hanoi Management Board of Export Processing and Industrial Zones under organization of General Manage

Once the following cases happen, victim or involved personnels are directly or mainly responsible:

Violation of operation guide or risky working, which causes accident.

Violation of safety procedures or work processes, which causes fatal accident

Violation of labor regulations , arbitrarily open / turn on the device or arbitrarily change , disassemble , damage , misuse of equipment and safety equipment, which causes accidents ;
3.8 Once the following cases happen, in-charged person is responsible:

Safety procedures/policies, accountability regime, operation procedures are not consolidate, employees don't have rules to comply with so fatal accident happens

Training on safety and technique was not conducted or unqualified employee perform work, which causes fatal accident

Machinery and equipment of which inspection certification expired are operated or machinery and equipment is overloaded or improvement action for deficiency of machinery and equipment is persisted, which causes fatal accident

The working environment is not safe while improvement measures are not applied, which causes fatal accidents;

The basic techniques or construction projects do not apply “Three the same”, which causes fatal accidents ( Same design / same building / same using)

Based on the severity the accidents, different levels of penalty for the responsible person will be carried out, the penalties include administration, economy and criminal
.
3.10
Conduct monthly EHS meeting or during between OHSAS meeting between management and worker representatives to assess the number of accidents, status of loss, status of accident to analyze and summarize.

The company firmly eliminates cases of which purposed sabotage were proved and had clear evidences.

Accident prediction and warning
T horoughly grasp safety management guideline "Safety first, prevention is primary” which is represented in new safety concept "Think safely, act safely". Through OHSAS-risk assessment to find risk prevention measures and address hazards, based on human, machines, materials, laws and environmental principles to find out potential hazards in the production process, analyze and identify hazards, apply preventive measures, and simultaneously conduct risk assessment of construction or machinery before they are put into use. Enhance awareness of workers against dangers of worksites, improve their ability to recognize and prevent danger, thereby reaching the milestone to share experiences and techniques, “I know safety, I understand safety”, develop safety practices and the ability to recognize accident prevention arising . |
